The state Department of Health has taken the following disciplinary action against health care providers.
The department regulates the practice of health professions and enforces health and safety laws to protect the public from negligent, incompetent, or illegal health care providers.
To place a complaint against a health care provider or get disciplinary information call the Health Professions Quality Assurance Division, (360) 236-4701.

In October 2002, the Unlicensed Practice Program Issued a Cease and Desist Order against Ralph G. Graham.
Graham must cease and desist from practicing as a chiropractor in the state of Washington without a license.
