Two Lyle residents were taken to Emanuel Hospital in Portland following a three-vehicle accident on June 2, 2005.
The accident occurred at approximately 7 p.m. on State Route 14 just east of the Lyle tunnels.
According to the report, a 1998 Jeep Cherokee driven by America Y. Perez, 15, of Lyle, was westbound on SR 14. She was following a 2003 Volkswagen Jetta driven by Christopher C. Hooper, 46, of White Salmon. A 1996 Peterbilt tractor driven by Richard J. Hoefert, 51, of Milwaukee, Wisc., was traveling eastbound on SR 14.
Perez's vehicle crossed the centerline into the oncoming lane. The left front of her Jeep struck the front of the Peterbilt. The vehicle driven by Hooper was damaged by debris.
Perez suffered a broken left leg, pelvis, arm and had a head laceration. She was transported to Emanuel. It was unknown if Perez was wearing a seatbelt.
Neither Hoefert nor Hooper, who were both wearing seatbelts, were injured in the accident.
Two passengers in the Jeep were also injured. 41-year-old Magdalena Velasquez, 41, of Lyle, suffered a broken hip and nose. She was also transported to Emanuel. Victoria Mendoza, 62, The Dalles, Ore., suffered minor injuries. She was treated and released at Mid Columbia Medical Center in the Dalles. It was unknown whether the two were wearing seatbelts.
